I don't think there is any issue using 'standard' RF licensing for a book or movie cover. The only time an extended print license is needed is if the photo or illustration is the featured item. Things like a photo mug, calendar, etc. are all items that require an extended license, and all are examples of things where you buy the item because you want the photo. A book or movie isn't like that...

An extended print license isn't about whether you are printing the image, it is all about whether the image is 'essential' or not and is considered redistribution. A book cover is an example of a specific use PERMITTED under the standard RF license.
